EVV - Creating the Aide Care Plan


This process creates the aide care plan, including:

  • Days of the week & times the aide will visit.
  • Tasks to be completed.
  • Generates the schedule for:

    • EVV Visits Screen
    • Schedule Calendar
    • Care Champ App & Telephony
1. Steps to Create an Aide Care Plan
2. Go to Aide Activity > Aide Plan and click New.
3. Set Authorization Dates - Adjust the From and Thru dates to match the authorization. (Auth dates appear at the bottom of the screen.)
4. Assign Staff & Verify Payer Information
  •     Select the assigned aide and office case manager.
  •     For CAP/PCS: Make sure the three-digit payer and job code match—this is vital for billing & EVV data submission.
  •     Vst Code should be a regular chargeable visit.
5. Select All Applicable Tasks - Assign tasks based on the service plan (days assigned in the next step).
6. Assign Visit Days - Check the boxes for each day of the week the aide will visit.
7. Set Visit Time-Adjust times as needed to match the total authorized time as closely as possible.
8. Modify Task Assignments (If Needed) - If certain tasks should not be performed on a specific day, uncheck the box for that task on that day.
9. (Optional) Add Precautions - Click the Precautions tab to select precautions or other indicators.
10. (Optional) Add Special Instructions - Enter notes under Special Instructions as needed. This will show up on the Start screen of the CareChamp visit. 
11. (Optional) Save to Print/Preview - Verify information before proceeding.  

12. Finalizing the Schedule - Select the Plan & Click Edit
13. Click Create Schedule and wait for process to finish. Save Plan.
⚠️ This step generates the aide/patient schedule, creating visits for Care Champ & Telephony.
14. (Optional) Verify the Schedule - Go to Aide Activity > Aide Schedule and pull up the patient.
  • Use Month View to adjust minutes as needed to ensure the monthly hours match the authorization.
