02.58 OASIS Visit Assessments


This report pulls a list of OASIS type assessments - Start of Care, Resumption of Care, Recertification, Discharge.  See below for all the information this report includes.

How to Run the Report

From Barnestorm Office, click on Reports > Visits > 02.58 OASIS Visit Assessments.

o   Starting Date = Enter the beginning search date.

o   Ending Date = Enter the ending search date.

o   Team = You can enter a team number(s) or leave blank to select all.

o   Program = You can enter a program number(s) or leave blank to select all. 

o   Payer =  You can enter a payer number(s) or leave blank to select all. 

o   Employee(s) = You can enter an employee number(s) or leave blank to select all.

o   Assessment = You can enter an assessment type(s) or leave blank to select all.  You will need to type in the full description.  Here is an example.  If you want all SOC and ROC assessments you will need to type this in the assessment field: Start of Care, Resumption of Care.

o   Status 00, 01, 02, 03 = You can enter an assessment status(s) or leave blank to select all.  00 = Incomplete assessment; 01 = Completed assessment; 02 = Locked Assessment; 03 = Locked and Charged.

How to Read the Report

o   Chart = Patient chart number.

o   Patient = Name of patient.

o   Tm = Assigned team number.  This is entered on the employee tab of the Referral.

o   Date = Date of assessment.

o   Pro = Program number assigned to the assessment.

o   Payer = Payer number assigned to the assessment.

o   EE# = Employee number (of the employee) who created the assessment.

o   Employee = Name of employee who created the assessment.

o   JobCd = Job code assigned to the assessment.

o   Assess = Type of assessment

o   Visit Type = Type of visit, based on the visit status code assigned to assessment.

o   Visit Status = This column indicates the visit status:  incomplete, complete, locked, or locked&charged.
