Hospice CAHPS Caregiver Details


This is a feature to collect Hospice caregiver information for CAHPS export.  This screen will only appear when a Hospice patient is selected. On the Select Patient screen, select the patient. Then click on the Background/Envnmt tab > Environment tab > Hospice CAHPS Caregiver Details tab.

From here you have two options:  

1. At the top section of the screen you will have option to "disqualify the decedent from the CAHPS survey. See notes below.
2. At the bottom section of the screen, you will have the option to include the caregiver in the survey. See notes below.

Patients/Caregivers NOT being sent on the survey:
Of the six options listed, select the reason why the decedent/caregiver will be disqualified for the survey and click the Save CAHPS Data button.  After you select a reason, the bottom portion for caregiver information will disappear.  If you did select a reason by mistake, you can right-click it to deselect the item. The caregiver questions at the bottom will then reappear. 

Patients/Caregivers that WILL be sent on the survey: 
The items with yellow boxes are required before saving.  Select pieces of information will be copied forward from the patient's referral.  The caregiver fields from the referral will not copy forward due to irregular use of these fields.  NOTE: The caregiver and emergency contact information, at the bottom of the page, is included so that you can copy and paste select information as needed.   
  • The patient's last name is copied to the caregiver's last name
  • The patient's address will be blank, with an option to "Copy the Patient's Address"
  • The language is defaulted to English
  • The facility name of your agency will automatically be filled in
  • "Patient is a US Military Veteran" will default to No
  • If the discharge reason is "09 Expired at Home," the "Last Care Location" will default to Home
Click the Save CAHPS Data button when finished.  If any required fields are blank, you will need to answer them before information will be saved.
