Change the Patient on a Care Coordination Note


If you accidentally entered a Care Coordination Note under the wrong patient, you can now change the patient within the Care Coordination screen.

o From the Select Patient screen pull up the patient that currently has the care coordination note. 

o Click on the Care Coordination tab from the Main Menu.

o Click on the View and/or Select for Edit tab at the top of the screen.

o Click on the All button and select the CCN that needs modified.

o Click on the red Edit button at the bottom of the page.

o From the top, change the patient’s chart number or search for the patient by name.

o Click on the Save button.

o A message may pop up asking if zero minutes is ok.  Click on Yes or No.

o A message will appear asking if it’s ok to change the patient on the CCN.  Click on Yes
