HIS → HOPE: Assessment & Quality Measure Reports Effective Oct 1, 2025

Where to submit records and run reports.

Key Information (HIS → HOPE):

QTSO: What to Expect for HOPE in iQIES
  • HIS target dates before Oct 1, 2025: submit in QIES through Feb 15, 2026 (no new/corrected HIS after this date).
  • HOPE submissions: in iQIES only.
  • As of Oct 1, 2025: submit HIS & HOPE deletion requests in iQIES; HIS deletions are processed in QIES once approved.
  • Move requests (HIS/HOPE): continue via the manual forms on iQIES Help → Patient Assessment Move/Delete Forms.
Examples of when to use HOPE vs HIS 
HOPE Guidance Manual Change Table v1.00 to v1.01
  • Discharge on or before Sep 30, 2025: Submit HIS Admission + HIS Discharge.
  • Admit on or before Sep 30, 2025; Discharge on or after Oct 1, 2025: Submit HIS Admission and HOPE Discharge (no HUVs).
  • Admit on or after Oct 1, 2025: Submit HOPE Admission, HOPE Update Visit(s) (if triggered), and HOPE Discharge.

Barnestorm Steps - Labels that say 'Hospice Item Set' will change to 'HOPE' later 

Where to document

  • HIS and HOPE are documented in the same location in Barnestorm (no changes to menu location).
  • Selecting the Hospice Item Set screen will ask if you want to use the HOPE or HIS feature when patient's admit date is prior to 10/1/2025. 
  • The HOPE Grid will trigger records that are due; ie. HUV1 and HUV2 will appear on and after the date due.

NEW → Validation for HOPE Assessments

  • Barnestorm can direct you to the Validation Utility Tool (VUT) before upload to catch errors early.

Where to export

  • Create the file the same way as before: Admin → Export Hospice (HIS).
  • HIS can be submitted using QIES and HOPE through iQIES. (Barnestorm does not provide submission steps.)

Where to run QM reports (not in Barnestorm) 

Now — CASPER/QIES (QM Reports)

  • CASPER → Reports → Hospice QRP
  • Run: Review & Correct, Hospice‑Level QM, Patient‑Level QM, Provider Preview
  • Use weekly for data hygiene & previews

Oct 2025+ — iQIES (Submissions & Provider)

  • iQIES → Reports → Find a Report
  • HOPE submission/validation + provider reports
  • QM reports move here starting Feb 2026

Measure set at‑a‑glance

  • ✅ CBE #3235 — continues; now calculated from HOPE items.
  • 🆕 Timely Follow‑up for Pain Impact — SFV within 2 calendar days after first moderate/severe pain impact (at Admission or Update).
  • 🆕 Timely Follow‑up for Non‑Pain Symptom Impact — SFV within 2 calendar days after first moderate/severe non‑pain impact (at Admission or Update).
  • ✅ HCI & HVLDL — claims‑based; no change.
  • ✅ CAHPS® Hospice — experience survey; no change.
