How To Run the Report

• Barnestorm Office: Click Reports > Patient > 22.06 Therapy Only Patients.
• Include DC Patients On/After: This allows you to include patients that have been discharged. Type in your selection date.
• After you've made your selection above, click Print the Report to view the report.

How to Read the Report

• Chart #: The patient's assigned chart number.
• Patient: The patient's name as keyed in the referral.
• Admit Date: The date of admission.
• Discharged: The date of discharge. If patient is still active, this field will be blank.
• SNDate: The date of last SN visit.
• SNvx: The number of SN visits.
• PTDate: The date of last PT visit.
• PTvx: The number of PT visits
• OTDate:The date of last OT visit
• OTvx: The number of OT visits.
• STDate: The date of last ST visit
• STvx:The number of ST visits
