▶ How to Run the Report

• Starting Admit Date and Ending Admit Date: Enter your starting admit date and ending admit date.
• Programs/Payers/Teams: Filtering by codes or leave blank to select all.

📄 How to Read the Report

• Chart#/Patient: The chart number and patient name.
• Program/Payer/Description: The program number and payer associated with the authorization.
• Admitted: Date of admission.
• D/C Date>: The date of discharge. If patient is not discharged, this field will be blank.
• AuthFrom/Auth Thru: The starting and ending date of the authorization.
• AuthNum: The authorization number. If there is no authorization, “No Authorization” will appear in this field.
• Per: The discipline P for PT, O for OT, and R for SN.
• LastVisit: The last visit date.
• NumVx: The number of visits made during your From/Thru dates.
• Hours_Vx: The number of hours/visits approved.
