06.24 Patient Counts by County/Age


This report will help you to keep track of patient counts by county/age. This is an unduplicated count, meaning the patient will only be counted once during your From/Thru dates.


How to Run the Report


From Barnestorm Office > Reports > Agency Data > 06.24 Pt Counts by County/Age


  • From and Thru Dates = Enter your starting search date and ending search date.
  • Program(s) / Payer(s) /Team(s) = Enter specific program(s), payer(s) and team(s), or leave blank to select all.
  • Admits Only = Only admissions will be counted.
  • Duplicated = This will count the patient for each chart#.
  • Age Ranges = By default these ages will be counted: 17, 40, 59, 67, 74 and 84.
  • Calculate Age on Date of First Visit During Reporting Period = This calculates the patient age on date of first visit of the reporting period.


After you've made your selections above, click Print to view the report. The additional print options include Landscape and Export to Excel.


How to Read the Report


  • Cd = The county code assigned in the patient's referral.
  • County = The county name/description.
  • Age Range = The age ranges indicated.
  • HH = The patient count for Home Health.
  • CAP/PCS = The patient count for CAP/PCS.
  • Hospice The patient count for Hospice.
  • TotalThe total patient count for that county/age range. 
  • # of HH Visits = The total number of visits for that county/age range. 


NOTE: If you see the patient count is not accurate, please use the following reports to make sure there is no missing county codes or missing/invalid date of birth in the referral.
