This report will show you the date of the last HbA1c keyed into Barnestorm and it will calculate when the next one is due. It prints active patients that have diabetic ICD codes.
How to Run the Report
o Go to Barnestorm Office > Reports > Audit > 07.09 HbA1c Tracking
o Check the box Show Case Manager instead of Payer if you prefer
o Program(s) / Payer(s) / Team(s) = Filter report by entering program, payer, Team number(s). Or leave blank to select all.
o Click the Print button
How to Read the Report
o Last HbA1C = Shows the last result date keyed; if blank then no results found
o Next Date = Calculates when the next HbA1c is due; if blank then no previous results found
o Diagnosis = Related diabetic ICD code
