Effective April 1, 2014 your CMS1500 claims will look a little different, due to the revised version of the CMS1500 form (02/12) that replaced previous version 08/05.  The version was changed (by CMS) in anticipation of the upcoming ICD 10.

NOTE:  If you are sending claims to payer that requires the old format:  On the Print UB04\1500 screen you will have the option to Print Old Version (08-05).  Check that box before printing the claim.
