Additional Documentation Requests (ADR) Tracking


Note: the UTN is required before you can create the EOE claim, so in addition to the tracking below, please add the UTN on Billing > PPS Billing > Edit PPS Episodes.

For ADR Tracking, from Barnestorm Main Menu, select Audits > ADR Tracking.  This screen will appear:

On the Main tab is where you will pull up the patient.  Type in the chart number or search by last name.  To minimize the listing that appears, there are two alternate selection/checkbox options:  1. "Only Search for patients in Program #," or 2. "Include D/C Patients."  Once you select the patient, from the listing, the payer field will populate (beneath the patient field) and a list of episodes will appear beneath the "Select the 485 period for the ADR."  (See screen shot below) Select the appropriate payer and the appropriate 485 period, then click Save.



(If there is an asterisk by the episode period, this indicates there is an ADR Tracking established for that 485 episode.)  On this same screen, you will enter the dates to track the ADR period, which includes the Date ADR Received; Date ADR Due; Date ADR Mailed; and Date ADR Resolved.  You can print the reports by using the Reports tab.  From there you can either Print all ADR History or Print All Unresolved ADRs.

From this Main screen, you can also add additional notes or comments under "Items found in the paper chart that are not in Barnestorm," "Notes," and "Final Status."  (Make sure to hit Save, whenever any data is entered.)

We recommend using Documents (if you have this Barnestorm service).  We have created an ADR folder for each patient.
Place the documents into the ADR folder when you bring them up for printing, as shown below:

There are also options to Print ADR Tracking Sheet; and to Delete the Tracking Period (if entered in error or the wrong 485 period was chosen).

We recommend using the ADR Checklist documents to track what to send. Those are linked at the bottom of this article, and for any Barnestorm customers that use FormFiller, they have been added for you to check off, complete, and file for the patient within FormFiller in Barnestorm. If you have Documents but not FormFiller, you can scan the checklist to the patient's ADR folder. If you'd like to add Documents and FormFiller to your Barnestorm service, please give us a call at 877-999-1171.

Other SubMenus of the ADR Screen include:

Visit Notes = Includes all Notes for the patient, during the 485 period.  You have the options to Show Visit Status; and Order Visits by Date or by Discipline.  You also have various print options.  (See below)



OASIS = Includes all OASIS documents for the patient, during the 485 period.  You have the option to preview/print the long or short form.  You also have the OASIS Compare tab to compare previous OASIS.

Verbal Orders = Includes all Verbal Orders for the patient, during the 485 period.  Print List of Orders will print the running list on the screen.  Print All Orders will print the actual order preview.  Check Include FYI Orders to print those as well. 

Care Coordination Notes = Includes all CCN for the patient, during the 485 period.  There are multiple print options on this screen.  (See below)



Reports = You have the option to Print All Unresolved ADRs or Print All ADR Requests.

ADR Denial Tracking
Track denied ADR charts and print reports to show previously denied ADRs and current tracking.
