Medicare Denial: 30720 Treatment Authorization


Reason code 30720 indicates that the treatment authorization is either not present on the final claim, or is not valid.
The treatment authorization code is not present or not valid. The treatment authorization code needs 
to be eighteen positions and in the following format for episodes 01/01/2008 or after: Position 1 and 2 = numeric Position 3 and 4 = alpha Position 5 and 6 = numeric Position 7 and 8 = alpha Position 9 = numeric Position 10 = 1 or 2 Position 11 - 18 = alpha

Check the OASIS. Print the short form. If the treatment code is not present at the top or not according to the format above, that's the issue.

Check to see if the RAP has paid.

If the RAP has not paid:

  1. Unexport OASIS.
  2. Unlock OASIS.
  3. Edit OASIS.
  4. Change corr# to 01.
  5. Verify.
  6. Lock.
  7. Print preview, make sure authorization is there at top.
  8. Cancel the RAP and create it again.


If the RAP has paid:
  1. Cancel EOE.
  2. Cancel RAP.
  3. Unexport OASIS.
  4. Unlock OASIS.
  5. Edit OASIS.
  6. Change corr# to 01.
  7. Verify.
  8. Lock.
  9. Print preview, make sure authorization is there at top.
  10. Cancel the RAP and create it again.
