Code for Transfer From Another Agency: Condition code 47

Condition Code 47 is required, on the RAP/NOA, when the patient is transferred from another HHA (within that agency's episodic period). If the code is not on the RAP/NOA (of the receiving agency) the claim will deny, and may cause your RAP/NOA to be untimely causing payment penalties for your agency.

** It is important to verify this at admission: If eligibility shows an existing HH Agency on file, condition 47 needs to be put in the Referral. Otherwise, the NOA will deny due to overlapping home health admission. Here are the instructions on getting the condition code 47 onto your RAP/NOA claim:
