Enter an OASIS in Barnestorm Office


    Note: From Barnestorm Point of Care, you should ALWAYS build the OASIS from your VISIT. The process shown below is for Barnestorm Office users only, NOT for clinicians who use Barnestorm POC.

  1. From Barnestorm Office, select the patient from the Select Patient screen.
  2. Click on the OASIS tab from left column.
  3. Click on the New button to start an OASIS.
  4. Select the MO100, Reason for Assessment and change the MO090 date then click on Create OASIS.
  5. From the first screen change the employee to who completed the OASIS as needed.
  6. Change the MO090 date as needed.
  7. Answer the MO032 as needed.
  8. Answer the MO110 and MO826 (M2200) question.
  9. Click on Next from the top of the screen.
  10. From here, you will be taken thru each OASIS question by using the Next button.  If you need to go back to a previous question you can click on the Previous button or select the MO question from the list in the left column.  
  11. The skip patterns are built into the answers.
  12. The detailed question appears at the top of the page.  The answers follow below it.
  13. To answer a question click on the bullet next to the answer.
  14. There are two different bullets, circle and square.  The circle bullets mean that only one answer can be selected.  The square bullets mean that you can mark all that apply.  When you select a circle bullet the next MO question will automatically appear.
  15. If you try to skip a question a warning message will appear.
  16. With the ICD code questions, you can pull ICD codes from the ICD History screen.  This will find the correct ICD codes and pull them in order on the OASIS.  The severity will pull over as well if they are answered in the ICD History screen.
  17. If the ICD codes are not in the correct order, you can edit the ICD History by clicking on the Edit ICD History button.
  18. After the correct changes have been made click on Save then Exit.  Click on Pull From ICD History button again.
  19. On the last page, after entering all the OASIS answers, click on Mark OASIS as Completed, Not Locked. 
  20. A list of missing MO questions, if any, will appear.  Click on MO question from the left panel to go straight to that question.
  21. After answering any missing questions you must Review and validate the OASIS.  Do this by selecting the OASIS from the main OASIS screen and then clicking on the Review + Validate button.  This may take a few seconds to run depending on the internet connection. One of three things will happen:  a)  A pop up will appear letting you know no errors were found.  b)  A validate report will appear giving you warnings but also marking the OASIS as validated.  c)  A validate report will appear giving you errors which will prevent you from proceeding until all errors are fixed. As needed, fix the errors and then try validating the OASIS again.  Note:  The warnings and errors on the validate report come from CMS, not Barnestorm.  
  22. After the OASIS has successfully been validated with a date you may lock the OASIS so that it can be exported.  

Tips:

  • The OASIS page will show the status of the OASIS: New = OASIS entered, not completed; Not Locked = OASIS completed, not locked; Locked, Not Exported = OASIS has been locked but not exported; Exported = OASIS has been exported and shows the date.
  • The OASIS page will show the date and time that the OASIS was last modified. 
  • Barnestorm prevents you from entering an OASIS out of sequence.  Ex.  You cannot enter a transfer OASIS until the resumption OASIS has been entered.
  • You do not have to open the OASIS to lock it.  You can use the buttons from the OASIS page.  Once you Finish and Verify that the OASIS has no errors, you can Validate and Lock it.
