Customize Care Coordination
Overview
- Customize the content used in the Care Coordination note (Topics, Who, and Text Item Box).
- Best managed by clinical directors or designated staff with appropriate permissions.
How to Access CareCoord Note Codes - Topic / Who - Go to Barnestorm Office > Codes > POC Codes > Note Codes.
- From Filter Page, choose CareCoord.
- Use the Name drop-down to pick which part of the Care Coordination note you want to change.

Text Item Box
- Stores frequently used conversation text as reusable items.
- Staff can insert the saved text into the note with a single click.
- Useful for standard messages such as:
- Routine coordination updates with family members/staff.
- Standard follow-up instructions or education notes.
- Common communication phrases used by your agency.
IMPORTANT: Changes to CareCoord Note Codes affect all Barnestorm users in your agency.These updates should be made by clinical directors or by clerical staff with sufficient permissions, at the direction of the clinical leadership.
Tip: When rolling out changes, notify staff so everyone understands new Topic, Who, and Text Item options in the Care Coordination note.
