The Source of Admission is listed in the Start screen of the Referral.  Below is a list of the SOA codes.  Pressing the "drop down" tab by the source of admission field will pull up this same list.

Source of Admission Codes are as follows: 

o 1 – Nonhealthcare Facility Point of Origin (previously Physician Referral) 

o 2 – Clinic or Physician's office referral

o 4 – Transfer from Hospital

o 5 – Transfer from SNF or ICF

o 6 – Transfer from Another Health Care Facility

o 8 – Court/Law Enforcement

o 9 – Information Not Available

Use of condition code 47 is required when a patient is transferred from another HHA.  See KB article: When To Use Condition Code 47?
